ALL STUDENTS REQUESTING A WORK PERMIT
Follow these simple steps:
v GET A JOB OFFER FIRST, then
v Obtain a B1-1 Form, “Statement of Intent to Employ Minor and Request for Work Permit” from the Work Permit office at your school or district office. Work Permit Application Forms
v Fill out the form completely and properly (PRINT); obtain signatures from your employer and parents.
v Return the application to the Work Permit office at your school or district office.
v Most work permits can be issued when you turn in your B1-1 form, but different districts have different policies.
v During the summer or any vacation time the high school work permit office is usually closed and if you need a work permit you will need to contact the District office. Someone there will be able to assist you. If you are visiting the area for an extended period of time during the summer, the local school district would be able to issue a work permit. You will need to bring documentation verifying your age. The permit issued in the summer would be a vacation permit.
